📌 Required Columns
  • first_name — Patient's first name
  • last_name — Patient's last name
  • date_of_birth — YYYY-MM-DD, MM/DD/YYYY, or MM-DD-YYYY
  • gender — Male / Female / Other
📌 Optional Columns
  • middle_name, blood_type
  • contact_number, email
  • address, city, province, zip_code
  • emergency_contact_name, emergency_contact_number, emergency_contact_relation
  • height_cm, weight_kg
  • allergies, medical_conditions, medications
  • notes, status
💡 Tips
  • Column headers are matched by name (case-insensitive)
  • Download the sample CSV template for reference
  • Comma and semicolon delimiters are supported
  • Maximum file size: 5MB
  • Duplicate rows are not automatically detected
